Access to the Health Records of a Deceased Patient

The Access to Health Records Act (AHRA) 1990 provides certain individuals with a right of access to the health records of a deceased patient. You have the right to ask for copies of either the whole or certain parts of the patient’s records from 1991 onwards.

Only the following people may apply for access to this information:

  • The patient’s personal representative. A personal representative is the executor or administrator of the deceased person’s estate.
  • Any person who may have a claim arising out of the patient’s death.

We appreciate that this is likely to be a distressing time for you and we want to do what we can to make the process of applying for this information as stress-free as possible. 

Keeping personal information confidential and secure is extremely important to us. As well as completing the form below we ask that you provide us with proof of identification and current address, to include two items from the list below:

  • copy of the identification/photographic page from a current passport;
  • copy of the identification/photographic section of a current driving licence;
  • copy of the identification/photographic section of a concessionary travel pass;
  • copy of a recent utility bill or bank statement (with financial details blanked out).
