Multiple InPhase reports being submitted by crews in the D&G West area with very poor or nil follow up. What is the reason for the poor response? Are the InPhases being closed off locally and not received appropriately? As this is the reporting interface for the crews to highlight areas of concern, it is imperative for awareness, transparency and understanding, that the staff receive the appropriate response.

The Scottish Ambulance Service uses InPhase as its incident, adverse event and risk reporting system. All staff are able to submit reports directly into InPhase, and once submitted these are visible within the system and allocated to the relevant manager or reviewing officer, depending on the nature and category of the report. Please see the table below detailing the total number of Inphase incidents reported in Dumfries and Galloway over the past twelve months and how many of those have where the original reporter has received feedback; this has been broken down by station:

The figures provided reflect feedback recorded within the InPhase system only. In some cases, feedback or reassurance may have been provided verbally or through informal discussion and not recorded within InPhase. Where feedback is not recorded in the system, it will not be captured within these figures.
It should also be noted that not all InPhase reports will necessarily require individual feedback to the reporter. Depending on the nature of the report, the information provided, and whether any further action or learning is identified, a report may be reviewed and closed within the system without direct feedback being sent to the individual who submitted it. For example, this may include reports where the information is noted for monitoring purposes, duplicates an existing issue, or does not identify any additional actions beyond existing controls.
The Service does not hold recorded information that explains the reasons for the perceived quality or timeliness of follow‑up in a specific geographical area, nor does it hold records that assess or evaluate the adequacy of responses provided to individual InPhase reports.
FOISA provides a right of access to recorded information only. It does not require public authorities to provide explanations, opinions, or commentary, or to create new information in order to respond to a request.
In addition, the Service does not hold recorded information confirming that InPhase reports raised by staff in the Dumfries & Galloway West area are being “closed locally” without being received through the InPhase system. Once submitted, reports are retained within InPhase and are accessible to those with appropriate role‑based permissions.
Information relating to local management practices, follow‑up actions, or internal performance monitoring is managed operationally and is not held in a recorded form suitable for disclosure under FOISA.
