Before you make a booking, please consider the following:

Please remember to avoid raising the patient’s expectations – not everyone is eligible for ambulance support

Before contacting us, make sure you have:

  • Patient’s CHI number
  • Patient’s full name, home address, date of birth and phone number (either patient or responsible person)
  • A knowledge of the patient’s level of mobility
  • A knowledge of any access issues at the patient's house
  • Date and time of the appointment
  • Hospital and clinic the patient is attending
  • Type of appointment (for example, outpatient)
  • Any specific needs, such as language barrier, hard of hearing, infectious disease, mental illness)
