Requesting an ambulance to support you to your appointment
We provide ambulance care to patients who need support to reach their healthcare appointment, or for their admission to and discharge from hospital, due to their medical or clinical needs. Requests can only be accepted for appointments between 09.00 – 15.00 and up to 10 days in advance of the date of appointment
We are currently experiencing a high volume of calls for our Scheduled Care appointments and please ask for your patience during this time.
We would ask you please fill out the form below and our team will respond in due course. Any requests for appointments out with 10 days will not be processed'
On the day requests will not be processed using the form
When we call you, we will take you through an assessment to help us understand your requirements and if you need to use our service.
Please also note that call backs will be prioritised in appointment date order and will not necessarily be made on the same day as the form is sent.

In response to this pressure, we ask that patients explore all safe, alternative means of travel, including family and friends, prior to requesting ambulance support. A list of alternative providers can be found here.
Should you no longer require ambulance support to attend an appointment, please call us on 0300 123 1236 (option 2) and we will cancel your transport request. Please note that in these unprecedented times, we may need to regrettably cancel ambulance transport at short notice. Thank you for your understanding.
How to book your ambulance?
If your appointment is within the next 10 days, please use our online booking form above. Any requests for appointments out with 10 days will not be processed' and neither will on the day requests

You can also ask your GP or carer to make the phone call on your behalf.

Please note: you must request your ambulance transport by 12 noon the day before your appointment.
Please dial 0300 123 1236 during opening hours and choose option 3 for enquiries. Then follow the instructions to get to the area best placed to answer your enquiry.
